Request for Audit Rescore

Organizations who have received a Provisional Accreditation or Non-Accreditation decision may request an appeal for BHCOE to conduct a rescore of their evaluation, should they feel there was an error in the scoring of the evaluation. An appeal should be requested within 30 days from the date of the debrief call. Please complete the appropriate form below and send the file to compliance@bhcoe.org. Regardless of the year your evaluation was complete, you should choose the form that aligns with the BHCOE Standards of Excellence for Applied Behavior Analysis Services under which you were evaluated. If you are unsure, you may ask your evaluator.
